New Patients:

Submit –  Submit your demographic information in the light blue fields listed below (PPH will never share or sell any of your private information). We will then create a personalized HIPAA-secure Patient Portal for you. As soon as your portal is set up on our end, we will email you your registration link. Please allow up to 24 business hours.

Register – When you receive your patient portal registration email, simply follow and complete the registration process. Check your spam folder if you do not see your email invitation

Inform Us – Once registered and inside your portal, complete the patient questionnaire. The questionnaire is lengthy, but it is key to getting to the root cause(s) of your health issues and goals. Questionnaires need to be completed 24 hours prior to your appointment. Next, use your portal message center to upload any tests you wish to share: standard medical labs, diagnostic studies, or previous functional medicine tests can all be helpful. PDF format is required.

Schedule – For all appointments and appointment types moving forward, use the “schedule here” link below. (Please do not use the schedule link in your portal as it is not automated). Appointments are all virtual. We use doxy.me or Zoom; you do not need any type of account. Through video, we can safely complete your Functional Medicine Physical Exam, examining your nails, skin, eyes, and the inside of your mouth. Subsequent follow-up appointments and lab reviews are effective via both video or audio-only telemedicine appointments – simply choose your preference. We will conduct a preliminary review of the information you provide before your first appointment, at our convenience.  At the conclusion of your initial appointment, you will have many customized recommendations,  interventions, and a list of next steps for improving your health.
